Contractor Midwife Sourcing & Data Sharing

Purpose of This Consent
This form explains how MidwifePlus Ltd collects, uses, and shares your personal and health information to source and match you with a suitably qualified independent contractor midwife for in-home antenatal education and/or postnatal support.
By providing consent, you allow MidwifePlus to share relevant information with prospective midwives to enable safe, appropriate, and personalised care matching.

Nature of the Service

MidwifePlus operates a matchmaking and care-coordination platform.

Midwives providing services are independent contractors of MidwifePlus Ltd.

MidwifePlus does not provide medical diagnosis or treatment but facilitates access to regulated midwifery support.

Information That May Be Shared With Prospective Midwives
To assess suitability and availability, MidwifePlus may share limited, anonymised or semi-anonymised information with prospective midwives, including:

Medical and obstetric history (relevant to care provision)

Postnatal or antenatal needs and preferences

Personalised care request and proposed care plan

General location (e.g. postcode area only)

Household or safeguarding considerations relevant to safe care delivery

Cultural, language, or accessibility needs (where relevant)
Your identifying personal details will NOT be shared at this stage.

Information Shared Only After a Midwife Is Matched
Once you are successfully matched and a midwife accepts the assignment, the paired midwife only will be given access to your full personal details, including:
